• Patient Registration Questionnaire
Patients living within the practice area are welcome to register and our reception staff can confirm your eligibility from your postcode.
As a practice our first obligation is to provide a safe service to our registered patients and therefore the practice has decided not to accept out of area patients in view of the practical or clinical needs of our in area patients. This includes patients who are currently registered with us and moving to an out of area address.
You will also need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.
New patients are welcome to have a health check with a member of the healthcare team. Medical treatment is available from the date of registration. Please contact reception for further information.
You can print off a GMS1 registration form which can be found here. You can also download and complete our Patient Registration Questionnaire on your computer by clicking here. Once both of these have been completed, please bring them in with you on your first visit to the practice. Please note, if you have signed up for our online services, you will need to bring a copy of photo ID with you when you return your forms. This will enable our reception team to verify your identity in order to use online access.
Update (May 5th, 2020): in light of the ongoing coronavirus pandemic, we are allowing patients to register at the practice as well as sign up for online access without needing to visit the surgery. To register with us, please download and fill in the GMS1-Registration Form (you can then print, fill in the form and then scan it in order to send it back to us) and Patient Registration Questionnaire and email it to us at no-reply.spinney@nhs.net.
If you would like to sign up for online services, you will need to send a copy of photographic ID (e.g. a driving licence, passport) with these documents. Once we have received these via email, we will be able to register you at the practice and set up your online access. You will receive an email with your login details so you are able to use these services immediately.
