We are using a new online service called Register with a GP surgery that makes it easy to register with Spinney Surgery.

Fill in this quick Online Form to start the process. You do not need proof of address or immigration status, ID or an NHS number.

The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

Paper forms are still available if you need one.

You can check if you are in the catchment area using the tab below by entering your postcode.

We are accepting patients who live outside of the catchment area however, we will not provide a home visiting service if you are outside the catchment area.

Patients living within the practice area are welcome to register and our reception staff can confirm your eligibility from your postcode.

As a practice our first obligation is to provide a safe service to our registered patients and therefore the practice has decided not to accept out of area patients in view of the practical or clinical needs of our in area patients. This includes patients who are currently registered with us and moving to an out of area address.

The Spinney Surgery supports patients who wish to use a non-gendered title as well as non-gendered pronouns. If you do not wish to use gendered pronouns/titles or if you identify as non-binary, please let us know what you would for us to use when filling in your registration form. We do not require proof of identity, address or immigration status from patients wishing to register and we are able to register patients with no fixed abode or who are considered homeless.

Non-urgent advice: Did you know? 

You can register at the practice without needing to visit the surgery. To register, please download and fill in the documents above and then submit them to us using the form above.


To register, you can print off a GMS1 registration form which can be found at the top of this page. You can also download these documents to your computer using the links at the top of the page so you can print them at home.

Once completed, please return these documents to the surgery and a member of our team will aim to register you within 7 working days. If you’d like to sign up for online services at the practice, more information can be found by clicking here.

You can register temporarily with a GP surgery while away from home for work, study or on holiday. You’ll remain registered with your permanent GP surgery. Please be aware NHS primary medical care is free and accessible to all and reciprocal health care arrangements are not relevant.

To register as a temporary patient, please contact practice directly via our reception team. We will ask you to fill in a temporary resident registration form.

Please note that temporary residents are not able to sign up to use the surgery’s online services.
